Self-Register for a Questionnaire

Use this process when your organization sends you a self-registration URL to complete a questionnaire.

Step-by-step guide


To self-register for a questionnaire, perform the following steps:
  1. You will receive a URL link to the self-registration form. To go to the form, copy the contents of the link and paste it into your browser.

  2. Below is an example of the self-registration form. The name of the fields, and the selection of Roles, may vary depending on the configuration of the system administrator.

    2.  What do these fields mean?

      Employee Number – Your unique identifier for this company. This identifier could be an NPI, an employee ID, or something else entirely.
      First Name/Last Name – Your full first and last name.
      Email – Your Email Address for being contacted by this company.
      Department – The department or section of the company in which you are employed.
      Facility – The physical site or location in which you work.
      State – The state in which you work.
      Role(s) – The roles or duties you fulfill as part of your employment. Multiple roles may be chosen, if available.

  3. After filling out the self-registration form, confirm by clicking the 'Submit' button.
