Add an Attestation

Use this process when you want to add one or more general agreements to a questionnaire. Participants have to consent to each attestation by filling a checkbox before they can submit their questionnaire for review.

Step-by-step guide


 Show Me How to Create a New Attestation
To create a new attestation, perform the following steps:
  1. Click the title of the questionnaire

  2. Click the 'Attestation' tab

  3. Enter the attestation itself in the 'General Agreement' text field (You may also use the buttons in the text box to make formatting changing, such as font size, color, etc.)

  4. Click the 'Save' button to confirm

 Show Me How to Create Additional Attestations
To create more than one attestation, perform the following steps:
  1. Click the 'Add Attestation' button under Additional Attestations

  2. Enter the additional attestation in the 'Text' field (You may also use the buttons in the text box to make formatting changing, such as font size, color, etc.)

  3. Click the 'Save' button to confirm

 How does this look to participants?

The Attestation page is the final page in the questionnaire. Participants must consent to each agreement by filling a checkbox on this page before their questionnaire can be submitted for review.




Did you know: As with many other areas of the questionnaire, the attestation cannot be changed while the questionnaire is locked.
