Commercial Initial Setup

Health Endeavors is dedicated to making the setup process as seamless as possible.

Follow this step-by-step guide to get started with Health Endeavors.

STEP 1: Company Account Setup

Please send the following information to your designated Health Endeavors Account Manager.

  • Fill out the Client Intake Form with the following information:

    • Organization Legal Business Name

    • Organization Address

    • Organization City

    • Organization Zip Code

    • Security Official Name(s), Phone Number(s) and Email(s)

    • List of Any Participants (Hospitals,clinics, etc.) and Contact Information

STEP 2: TIN and NPI Mapping

Please fill out the following template with a list of the relationships between TINs and NPIs within your organization.

Download the TIN-Template to provide the following information for account setup. When completed email the form to your Account Manager.

  • TIN Name – The name of the TIN as it should appear in Health Endeavors

  • TIN Number – The TIN number associated to the TIN. 

  • Primary Care Practice or Specialist – This column allows a P, S, or it can be left Blank. P will designate a TIN as being a primary care practice. S will designate a TIN as being a specialist. A blank will skip a TIN in the patient assignment algorithm but utilize the TIN in the in-network configuration.

  • Associated NPI – The NPI number that is billing through the associated TIN. A Physician NPI can be associated to more than one TIN while a Facility NPI can only be associated to one.

  • Facility NPI – This column allows a YES or a NO to indicate an NPI as being a facility. If YES is used, the next column must be left blank for this TIN/NPI relationship and the NPI cannot be used in any other TIN relationships.

  • Primary Care Provider or Specialist – This column allows a P, S, or a Blank. P will designate an NPI as being a primary care provider. S will designate an NPI as being a specialist. a blank will skip an NPI in the patient assignment algorithm but utilize the NPI in the in-network configuration.


STEP 3: Secure Data Transfer

There are 2 initial options for transferring data to Health Endeavors.

  1. SFTP Connection Setup (Preferred)

    1. Download the SFTP Connection Worksheet

    2. Return the completed form to your Health Endeavors Account Manager

  2. Payer Portal Access

    1. Health Endeavors can be granted access to your payer portal. 

    2. To use this please provide Health Endeavors with Data Access to the Payer Portal.

    3. Please provide the following information:
      • Instructions to connect to the portal
      • The frequency in which files appear in the portal
      • A list of file types and naming conventions
      • All payer support contacts


STEP 4: Data Specifications and Requirements

There are some required files that Health Endeavors needs to get started processing your data.

  • Review claims document layouts as compared to the required Health Endeavors Data Elements to ensure your data is actionable.

    • If some required elements are not found in the claims documents, Health Endeavors will work with your team to ensure all needs are being met.

  • Send this list of files to Health Endeavors using the option chosen in the previous step.

STEP 5: HEDIS Measures (Optional)

If Health Endeavors will be tracking your organizations HEDIS Quality Measures, provide a list indicating which measures are needed.

  • Review the list of available HEDIS Quality Measures 
  • Desired HEDIS measures can only be updated annually.
    • Provide your Health Endeavors Account Executive with a list of quality measures your organization wants tracked in the following year by October 1st of the current year. 


STEP 6: Security Official Training

Next we will want to set up the account(s) for Security Official(s). They will be the main point of contact for Health Endeavors. The Security Official(s) will have the primary Health Endeavors account(s). They will be responsible for creating accounts for other members of the team, assigning roles, and organizing the initial management set up.
