MFT Access Setup

One of the first steps in setting up a Health Endeavors account is providing a way for our team to import your information. Access to the ACO MFT portal is our preferred way to access your data.



Contact your Health Endeavors Account Executive for a list of the information needed to provide access. You will need the following information:

  • Contact Name - The name of the contact who is receiving ACO-MS Access.
  • User ID - The User ID Health Endeavors uses to receive ACO-MS Data.

  • Email Address - The email address attached to Health Endeavors ACO-MS Account.
