PatientLookup.com - Medicare Patient Lookup

Patient Lookup is a powerful tool that gives providers access to a patients Medicare claims data at the point of care.


Step-by-step guide:


To use Patient Lookup, perform the following steps:
  1. Go to Patient Lookup page - https://www.patientlookup.com/


  2. Enter username and password. If you have a Health Endeavors admin account, enter the same login credentials to access Patient Lookup.
    Contact tim@healthendeavors.com or call 1-888-862-0366 to verify that your log-in is setup and ready to use.


  3. Enter patient first and last name or select "Advanced Search". Access the Patient History to review at the point of care.


  4. Click the Patient History "Show Me" Video to learn how to utilize all the features within the tool.  


Did you know:

  • If a patient is sharing their Medicare Blue Button Data, there may be two options for the patient. Either will work, but Health Endeavors recommends using the Population: ACO/Medicare patient when available.
